Who Qualifies for GAPP Home Care?
Children typically qualify for GAPP home care if they:
- Have active Georgia Medicaid (not pending)
- Have a medically fragile condition requiring skilled nursing
- Have a physician order documenting medical necessity
- Are under 21 years old
Common qualifying conditions include tracheostomy, ventilator dependence, G-tube feeding, complex seizure disorders, and other conditions requiring ongoing skilled nursing care.
